Simple solution. Ban offending teams from making field changes during overs for the next 90 overs they play when fielding - even across Tests.
If there's a left/right combo batting, then they can only set the field the first time each faces in an over. Thereafter, fielders must resume the same positions.
This could even be implemented during a Test. So each night, the match referee would make his decision.
